Dorothy - I am from CT - We do not put facility routine care on our careplans i.e
shower 2x a week, mouth care in AM and PM, nail care 1x a week, Braden scale
assessment on skin every quarter etc.... We do have our "routine care" and assessments
outlined in our policies and we have i.e. shower schedule on the CNA worksheet. We
have never had a problem with this type of care plan.
