This is the be all test:
Pull out your last RAP's.  Do they still describe the patient and the
current set of problems?  If yes, great.  If no, do a significant change.
If the RAP's still describe the patient as s/he is, next pull out your care
plans.  Do they accurately still describe (with perhaps minor changes) how
to care for the patient?  If yes, great!  Make a nurses note describing the
change and that you don't feel it's a significant change in condition.  If
the care plans do not describe how to care for the patient (including what
caused the change?), then you need to do a sign. change assessment.
