Title: Significant change

I had this discussion with a survey team and won the discussion.  A significant change is done once the resident stabilizes for at least two weeks.  I was not tagged after explaining to the team that with some therapy pts it would require a significant change weekly due to good progress.

I work in a Rehab facility that has many brain injuried clients who make significant changes, in improvement.  The head nurse does not feel a significant change needs to be done on this specific client who has made several gains in ADLS because this is an expected improvement.  I am saying this does warrant a significant change because it requires an interdisciplinary review and care plan changes.  The question is, do we or do we not do a significant change when it is an expected change?
