Subject: Significant changeI work in a Rehab facility that has many brain injuried clients who make significant changes, in improvement. The head nurse does not feel a significant change needs to be done on this specific client who has made several gains in ADLS because this is an expected improvement. I am saying this does warrant a significant change because it requires an interdisciplinary review and care plan changes. The question is, do we or do we not do a significant change when it is an expected change?

if the resident has plateued. If you are expecting the resident to
continue to improve based on the care and treatment currently being provided
then you would not be required to do a SCSA.
