I would put in a modification but would write somewhere why I did so.I wouldn't want a 
surveyor to think I was trying just to make the sentinel event go away.We don't doc on 
signs of dehydration esp with a gtube.Unless the patient couldn't tolerate the tube.
The MDS has very specific questions regarding dehydration and a good I&O protocol 
should be all that is needed.I find the more places you ask a nurse to doc on what 
they see,the more chances you have for inconsistencies.We in longterm care tend to 
over due with forms sometimes.What is clinically relevant is all that is needed 
usually.
