Why is a nitro drip needed?  If it is life sustaining the resident needs to
be in the acute care setting.  If there is another reason--intractible chest
pain (which we just experienced with a resident with Scleroderma)and nitro
is indicated but the resident is hospice or palliative, that may be a
different story.

I would not do a nitro drip in a NH.  First the patient needs to be on a
heart monitor, which most NH do not have, second, I would not trust most
nursing home LVN's to monitor the patient.
