MDS were a mess to put it mildly.  Example patient admitted in September ARD was 09/18/03, signatures on front were 01/30/04 and the close date 02/06/04.  Explain that to a surveyor!  Also they found an MDS ARD 12/19/03 that had been coded 8 on bathing.  When I looked at the CNA documentation, it clearly stated on 12/17/03 that person had got a bath and was a 4/2.  I copied that and then did a modification.  Luckily the surveyors knew that I had just started and I feel they were easy.  Obviously I got cited for MDS but I think it will be at an E level.  ANy words of wisdom to clear all this mess up would be appreciated.
Had the facility identified this problem in a QA/QI study? If so, what was their action plan to correct the situation? Was the data entered into the system before listed date? Is it possible that in doing a QA/QI study, completed assessments were found to be missing from the records? I have seen completed assessment with late dates for just that reason. Sounds like you have a really big mess to straighten out. Do they have an empty room for you to stay in while you are fixing the problem??? (just kidding) Good Luck!
