Some MDS Nurses chose to do annuals vs. sig. change.
Personally I do the Sig. change....but the team did capture the level of
functioning....etc...etc...sooooo....I would not do a thing.  In regards to
the RAPS...you or have her do a Global Narrative which would include the
various changes that have taken place, the items you may be putting in place
(or whatever).  Not every team member is required to complete the RAPS...one
person can do this.
