Care Plan states that the Foley will be d/cd once the wound is healed.  Signs and symptoms of UTI are also addressed on this Care Plan.  State Surveyor stated the Care Plan needed an actual date of when the Foley would be discontinued.  Come on!! Have these people ever been involved in direct patient care?
