THe American Health Informaton Association has published a LTC Practice Guideline which has been acknowledged by CMS as the "Professional Practice Guideline".
TheGuideline states, on page 44 ... "When pulling documentation forward to the new record a copy of the folliwng documentation should be placed in the new medical record: last MDS (if resident was expected to return from the temporary absence), advanced directive, social hisotry, immunization records, leisure interest survey, copy of last progress notes, preadmission screening documentation (PASARR)"

Does anyone have a reference to cite for what needs to be in a new chart
when the old one was closed out because the resident wasn't expected to
return from the hospital but did after all? I know of the MDS and RAPs
within 15 months. What else?
