Rena is correct, NEVER remove anything from a closed record. Why would you consider doing this? Can you imagine two years from now when the staff has been replaced and the closed record goes to court with no assessments and no one knows what happened to them? In SNFs we cannot remember why we did a sig change two months ago without specific documentation.

Therefore, If a resident is discharged and readmitted, the MDS from the prior admission must be copied and placed in the new record rather than removing the MDS from the prior record and placing it in the new medical record.
