The only thing we bring forward from closed chart that is original is any advanced
directives (DNR, LW, POA, etc). Medical records puts orignals in open chart and puts
copies in closed chart. She also puts copies of Dietary Assessment, Activity
Assesssment, Social Service Assessment, Wt. record so that we can compare current
condition to past (wt. gain/loss) etc.
