Okay, first off is this a Medicare A admission?  Next, what type of an
assessment and discharge did you complete?  If it was not an admission then
your d/c should be d/c prior to completion.  Then when they came back you
would not need to do a re-entry.  You would put the date of re-entry at A4a.
If they are Medicare then it would be re-admission assessment and start your
clock again with the re-entry date.  Hope this makes sense.
