Is this a state specific thing? We had a
"State person" tell us to do a reentry form after a discharge prior to initial
assessment. She did say that it was not required however we could do it
and put the date of reentry in sec A4a. (We have been doing this for about
5 yrs) She is no longer with the State and I was wondering if what she
told us is correct or should I call them again. I think it was just something
they preferred. I read the pages you referred to and it says it is not
required but I am still wondering if it is ok to do or should we
stop?

When the discharge tracking code is Discharged
Prior to Completing Initial Assessment, a reentry tracking form is not
appropriate on readmission to the facility (RAI User's Manual, p. 2-25).
The AB1 date remains the same regardless of the number of times the resident
is discharged with the tracking code Discharged Prior to Completing Initial
Assessment until the initial Admission Assessment (AA8a=1) is completed (RAI
User's Manual, p. 2-24).

Hi all,
I have a new admission that goes to the
hospital before the 14 day assessment. The appropriate discharge tracking
form is completed.When they return a reentry form is done and the MDS is
scheduled.The background info was gotten and completed by admissions on the
initial MDS.When the MDS is completed it will have the initial date of
completion for the background info. This comes up as an error stating that
the assessment is late according to the AB1 date.I have gotten 2 different
answers for this and would like your opinion as to how to avoid getting
these
errors.
