Put the date of reentry in sec A4a and see if the error goes away. We
usually do a reentry following a discharge prior to initial assessment (it's
just what we are used to doing) and put the date of reentry in A4a and don't
get the error when we do the admission assessment.

Subject: Reentry form or not?

Hi all,

I have a new admission that goes to the hospital before the 14 day
assessment. The appropriate discharge tracking form is completed. When they
return a reentry form is done and the MDS is scheduled. The background info
was gotten and completed by admissions on the initial MDS.When the MDS is
completed it will have the initial date of completion for the background
info. This comes up as an error stating that the assessment is late
according to the AB1 date.I have gotten 2 different answers for this and
would like your opinion as to how to avoid getting these errors.
