As far as coding
her transfers..... In order to code her as Independent , ALL transfers need to
be independent. If she is transferring some of the time w/ assist and
some of the time independently, code her as needing assist. You'll have to
determine how much assist she is requiring to determine if it is Limited or
Extensive......... As far as documenting reasons for going from one bed to
another, I can't offer much assistance as all of our beds are duly certified so
it's not an issue here. However, we do get an order to D/C from Med A ....
or to Admit to Med A, if they are being picked up after they are
already a resident. WE do not get order to admit to Med A if they are
being admitted from the hospital. Hope you find some of this

Question 2, I am updating my 672/802 (matrix and census forms) and I have a
resident that is supposed to have staff with her to transfer. She will
transfer self to toilet and to bed frequently. How would I code this,
independent or assist of 1/2?
