We have developed an admission assessment that addresses many of the MDS
questions ,the admission nurse does it.Our rehab eval addresses
cognition,vision,hearing,etc,also our H&P is usually done by the RNPs who
also address vision,hearing,cog,and dentition,etc.I look at all this info
when I do those MDS sections.If there is any question of what is correct I
redo the assessment myself and write a note.It is really great to have all
this documentation as I then can refer to it on my rap summary.Our speech
eval is also great for cognitive questions and dental[chewing].
