Do you all have rehab depts whose evals include some items for MDS use.I would suggest
that they do.Vision,hearing and cognition have a huge impact on how one proceeds with
rehab,goals,approaches,etc.We do a min mental which often patients score quite well
on,but problem solving and higher levels of how one manages life situations are
another matter.I didn't always proced on cognition but often do for our rehabs that we
are on the fence about sending safely home.I never believed that the the MDS
coordinator should be the only documentor.Patients are different with other
disciplines and other times of the day.I will e-mail our admission form to anyone who
