Thanks, I would like a copy of the form. I strongly agree that rehab. professionals need to be involved in sections of the MDS in addition to than C.,G., P. and T. Vision, cognition, nutritional status are some. Therapists also need to understand the whole puzzel, they are still task oriented and need to know the interplay between the various sections of the MDS, the entire RAI process and how this plays into payment and quality (OSCAR, Matrix, DAVE).
