It all depends on what your restraint assessment shows. Does the person use the siderail as an enabler? do they have any purposful movement? this how you would depend if a restraint or not. Educate the family and the resident on the risk vs benefits - write it up and have them sign it at least quarterly and then care plan it. The risk vs benefits have worked in my facility and the state likes it because it shows you are educating them
