When an ulcer is debrided, it remains an ulcer--not a surgical wound.  You would restage the ulcer since you can now see the wound bed.  Per page 3-160 "Debridement of anulcer merely removes necrotic and ecayed tissue to promote healing.  The ulcer still exists and may or may not be at the same stage as it was prior to debridement.  Good clinical practice dictates tht the ulcer be re-examined and re-staged after the debridement."

We just admitted a resident with a debrided decub (stasis gangrenous ulcer with a wound vac)

 

  1. Do I stage this as a decub or is it now considered a surgical wound?
  2. With the debridement complete…is it still considered to be an infection and coded in section I ?
