Hospice
usually provided their care plan, so I put a copy of it on top of our care
plan. Then, in my care plan, in one of the problems as to why they are on
hospice, I add a note to the problem saying they are on hospice, & add the
approach to follow Hospice care plan as one of my approaches. I would add
an approach to notify hospice of any change in condition to coordinate
cares
