I put the hospice care plans with ours, and I
also add hospice interventions to ours.
I have a general problem that states
"_____________ has terminal dx of _________ and is receiving hospice services".
I have the hospice staff sign the care plan signiature sheet and I invite them
to add interventions ad lib wherever they want. Hope this helps.
