We do the same thing. A few years back it was a focus during survey to show evidence that plan of care (both) were a team effort. We now do a significant change of condition, have Hospice represented at care conference along with family/representative and documentation takes care of itself. Sometimes the extra significant change seems like overkill but has worked well for us. IT helps to formalize all the little ins and outs of who’s responsible for what etc. Hospice also documents “team discussion” during their visit.

We pull a copy of their care plan for the medical record. We make sure the important areas on their care plan are addressed on our care plan. Our hospice group tells us they are required to have a care plan (of course) as are we, but our formats are very different, so we just make sure what needs to be incorporated is incorporated into our plan. "How does everyone out there incorporate the Hospice care plans with their care plans? Do you make sure you have each item from the hospice care plan included in your care plan....or do you pull a copy of the hospice care plan and include with your care plans?
Need help ASAP...we are trying to work on our POC for a not so pleasant survey and are under a tight schedule to get this corrected."
