We do the same thing.  A few years back it was a focus during survey to show evidence that plan of care (both) were a team effort.  We now do a significant change of condition, have Hospice represented at care conference along with family/representative and documentation takes care of itself.  Sometimes the extra significant change seems like overkill but has worked well for us.  IT helps to formalize all the little ins and outs of who’s responsible for what etc. Hospice also documents “team discussion” during their visit.   -----Original Message----- From: [EMAIL PROTECTED] [mailto:[EMAIL PROTECTED] On Behalf Of Holly McGran Sent: Tuesday, April 06, 2004 10:32 AM To: [EMAIL PROTECTED] Subject: RE: Hospice Care Plans   We pull a copy of their care plan for the medical record.
