First of all I would talk to my aids to see that they are actually coded correctly.  If not, I would write a clarification, stating the amount of assist needed, & keep it with the MDS.  If it is indeed coded correctly by your staff, it should be a "1/2" for supervision, plus physical help provided, but only one to two times during the last seven days.  Also, did he need assist just once on those particular shifts or did it happen more then once, because you could then code it "2/2".

Subject: section G coding???

I have a question about section G.  I know that an activity has to happen at least "3" times to code an item in section G.   We have 7 day flow sheets that we code ADL's on.   How should this situation be coded?
any activity, here it is walking in the corridor.
For all 21 shifts in a 7 day period, there are only two areas that indicate "2/2" for walking.  ALL OTHER shifts the entire week are "8/8" for activity did  not occur.  So this activity only occurred twice.  How do I code this?   "2/2"  or "0/2"   .  And for those PA rnac's,   there is one blank space!   So we don't know what actually occurred then.   Any thoughts?
