If the activity happened only once 8/8 is no longer appropriate. Like coding a 4, it is an all or nothing code.

For all 21 shifts in a 7 day period, there are only two areas that indicate "2/2" for walking.  ALL OTHER shifts the entire week are "8/8" for activity did  not occur.  So this activity only occurred twice.  How do I code this?   "2/2"  or "0/2"   .  And for those PA rnac's,   there is one blank space!   So we don't know what actually occurred then.   Any thoughts?
Thanks!  Nancy

I asked a similar question a couple of weeks ago and received many different responses. This does not seem like a situation that is covered by the instructions for coding. Rena perhaps you can shed some light as we are getting errors from our UMR team. They feel it should be 8/8 especially if the activity only occurred once. Clarification is appreciated. Thanks
