Regarding coding the fracture:  The acute code is used for the first episode of care and so if they are coming to your facility after being hospitalized with the fracture, then the aftercare code would be appropriate.  If it is a joint replacement, then you would use the v54.81 but if it is not, then the aftercare of a fracture would be the one to use.  And, there are v codes for aftercare of pathological, traumatic, or joint replacement.  If they are getting physical therapy and that is the reason for them being in your facility, then you would use the physical therapy code as a primary.  The primary is always the reason why a person is coming to your facility or staying in your facility.

It is always a good idea to check with your FI and make sure they accept V codes.  If not, please refer to the Coding Clinic Third Quarter 2000 as to what you need to do.  There is also very good information on using v codes in Coding Clinic 4th Quarter 1999.
