Regarding coding a fracture in Long Term Care.  Did the fall happen at your facility?  Were they hospitalized and if so, what was done?  A hip replacement?  There are aftercare codes that you would use or physical therapy codes if they are coming to your facility for physical therapy.  What is the primary reason for their admission to your facility?  If it is for physical therapy, then you could use V57.1 and the aftercare code in the V54's.  The acute code is reserved for the first episode of care unless there is any required continued management directly to the fracture.
