Are you talking about someone who is delusional?  Confabulate basically means to chat or to fill one�s memory with things that are not true. 

 

If the resident is demented or this is a long standing problem, you will likely be able to do nothing to reverse this trend.  I would use the goal that the resident would respond to redirection, etc.
