How about using Naomi File's approach. She prefers validation therapy rather than reality orientation. Use the who, what , why , where, how questions. For instance, if the resident tells you that he was a tight rope walker in the circus, ask .."where did you travel," "how did it feel to be up in the air," who did you work for." In the meantime, many years ago, I had just such a case and was told by a kind nurse "the resident is confabulating."  In reviewing the med hx, I found that indeed he was a circus performer!

Are you talking about someone who is delusional?  Confabulate basically means to chat or to fill one�s memory with things that are not true. 

 

If the resident is demented or this is a long standing problem, you will likely be able to do nothing to reverse this trend.  I would use the goal that the resident would respond to redirection, etc.
