In summary, the facility must then take the following actions:
1. Correct the original assessment,
2. Submit the corrected assessment, and
3. Perform a Significant Correction of a Prior assessment or Significant Change in Status assessment if the error was major, and update the care plan as necessary.
The Correction Policy Manual states on page 1-4.....
The traditional 7 day editing period following assessment or tracking form completion still plays an important role in the MDS process. The end of the 7 day time period remains important because that is the point at which the care plan is established or updated based on information in a completed assessment. If the record is not submitted and accepted by the end of the 7 day editing period, then a formal, paper audit trail must be maintained in the facility for any subsequent changes, until the record is accepted by the State. Any corrections after the editing period must reflect resident status and conditions as of the original Assessment Reference Date.

Since you have not yet transmitted the record, I would correct the record before it is transmitted. The flow chart on page 5-10 of the manual asks "record already accepted in Stete DB valid?" Your answer would be "NO". so then the next question is "is the record valid" your answer is yes. Then you correct the record in the facility and submit the record."
I would enter a note detailing when you found the error and how you corrected it, but would not submit an assessment that you know is in error and then have to go through a big paper and electronic trail to fix.

Subject: help with sig correction
Hi, I am not sure how to proceed with doing this correction.. so I need some advice from you.
I just noted that the dietician entered the weight of the 14 day mds at 298 when it should have been 198. In her notes, she does have 198 written. the care plan etc are accurate regarless of the wrong data entry. I have not yet submitted this, but it is over the 7 day period---so I am confused on what exactly I need to do?? This is what I am thinking...
1. I need to submit the 14 day locked assmt with the wrong weight
2. I need to do a correction--- but is this a major error--since the chart indicates 198 lbs???
3. if this is a major error, I correct the 14 day assmt , by doing a modification, then I must do another MDS as a significant correction of a prior assmt. do I use a new ard??? *--do I need to do raps again??
Please set me staight on this
