Why are we writing orders for "may" anything? Orders are Orders, you 'may' not have a choice.  You have to implement orders.

I would only count the "may ambulate" from your questions and I would only count that one IF the resident had been on ambulating restrictions and would now be allowed to ambulate.  The other scenarios do not really require a physician's order so should not be counted on the MDS.
