I agree with what you did, but I would have checked 6a as it is a foot problem, but would not have checked 6f as is is not a dressing.  6f is asking if you are putting a dressing on w/wo ointment.  Just my thoughts!  :)

I have a resident with intact blister to her left heel. We have an order to swab area with betadine and leave it open to air.

 I coded the blister on section M as a stage II pressure ulcer. I also checked 5e for ulcer care. Since we do not applying the dressing I did not check 6f.

I think I do not have to check anything else regards this blister, but my DON said I need to check 6a for foot problems and care.

Does she right?
