I am learning how to work the Raps and looking for some guidance. I would like any cheat sheets or worksheets anybody may have? And I am trying to figure the best way to set my ARD date. We have been using our 14 day as the initial assessment that means it has to be complete on the 14 day. Can I set the ARD for the 13 day so I have time to work the raps on the 14 day so I won't be late? Any pointers would help.

Thanks,

Lisa Pippin

Lisa,
I don't have any cheat sheets for the RAPs.  I do print up the last full assessment and use it as a "rough draft" for completing an assessment on each resident.  We make changes on the rough draft and can easily see if a significant change has occurred.  We usually to the Admission assessment with the 14 day assessment as well.  I try and set all my PPS assessments a day or two earlier than they have to be.  This gives us a couple of days to get RAPs done with the admission assessment.  Also, sometimes there is a reason a resident may fall short of a day of therapy (due to illness or holidays) and if this occurs you can back up the ARD date to capture more therapy days if necessary.  I've tried to do the Admission assessments with the 5 day pps assessment but prefer to do it with the 14 day since we know the resident so much better.
Hope that this helps.
