I would inactivate the assessment & re-do with the correct info since there is a LOT 
incorrect on this one. The unfortunate thing is: if it has been more than 14 days you 
are going to get default payment anyway - if I understand the rules correctly

Hi Gang,
      I have a question re: modification of assessment.  We have a patient that we did 
a 5 day last month, only to find out a month after that the ARD and the minutes were 
wrong.  Our fill-in MDS person did the assessment on exact 5 day and our rehab person 
gave her minutes based on 8th day( she used the 3 day grace).  Now my question is can 
I do a modification not just on minutes but also the ARD?  Can I change the ARD date?
