We have our admissions person talk to the case managers at the hospitals
and have it signed by the Dr before the res enters the facility. A copy is
usually faxed to us before admission so we know we have it.

THank you. When do you get this letter signed? According to the regs the
predictability must be written at the time of discharge from the hospital.
Does the hospital complete this?

Thanks
Ron

Here it is for all of you that asked for a fax. I finally got smart and
put it in my computer so I could e-mail it (thanks to a suggestion of a
member of the list serve!LOL!). Hope you find it helpful.

Faye, Same goes for me. 904-824-5458. I know this can be burdensome.
The
one time I offered to fax, I ended up sending out 64 faxes. My admin.
didn't say anything about the l.d. charges, but I bet he wondered.
Thanks. Charlotte
