Hi all,

Just wanted to report the most beautiful summer/fall day ever today!  Sorry for 
those in Phoenix with the flooding, but I decided to go to the lake and enjoy 
my day.  Took my  nook and a lounge chair, parked and enjoyed a great time!   
I'm thinking today through Wednesday here will be maybe our last good summer 
days.


Also, my count again came down on the bcr/abl.  Last test showed two weeks ago 
that my number is .35!!!  (Last test showed 1.70).  I am elated.  Also test 
results from the liver enzyme today showed I am almost back to normal in that.  
They were way up, and I was hoping I could go off the med for awhile, but got 
good news that apparently my body is getting used to bosutinib. (100 mg a day). 
 The ALT count was 68 and the norm is 52.  The ASP was in the normal range.  
Hooray!!!


Jeannie, I'm not happy to hear your sprycel is giving you problems.  I'm sure 
you will be well-researched about staying on it or trying another, etc.  It's a 
tough thing!  I have no others to fall back on, as you do.  So you can be 
thankful for alternatives!   I am so very thankful for the way all this is 
going for me.  Like Kareem Abdul Jabbar has said, "I will die of something 
else, but not CML!!!"  I think most of us are in that boat, thanks to the 
medicine. 


Praying also for Marty to receive a transplant soon!!!  All the rest of you are 
also in my thoughts and prayers.

Hi Susan and you are so welcome.  That Gleevec for my five years was   a trip 
no one want to go on, but we made it through, thanks heavens!  I   had the 
sores, rash, eye bleeds, hair loss and etc. but I didn't have   bone pain.  
This was a blessing for sure.  My onc at the time told   me one of his male 
patients had severe bone pain that he could barely stand at   all.  I did have 
awful cramps in my hands and feet.  And Gleevec   never put me into remission.  
My blood counts were all over the   place.  Finally after 5 years it just quit 
working at all.  I took   Tasigna that didn't work and am now on Sprycel.  
Sprycel finally put me   in remission, but have been losing that remission 
slowly as of late, and my   hair is falling out again.

In my case, this really affected me and Dr. Druker agreed with me to     get 
off of it after I already did…but then he suggested Nexium and I     absolutely 
refused.  Like I said, the Gleevec muscles spasms  are     quite painful to 
begin with…imagine that discomfort times 10 and that is     what happened to me 
two days after beginning the Prilosec.  Not going     there again!
    


    
I do not know if you truly remember me but I am the patient who is Dr.     
Druker's QUEEN for side effects…Dr. D. put me on his top 10% of all his     
patients with almost every reaction possible with Gleevec…the only thing I     
never had was the skin rash…just had eye bleeds, was transfusion dependent     
until the dosage was reduced to 250 mg, need every three month testing of     
the PCR and the PK levels.  In either case, besides the blood blisters     and 
the fragile skin, possible neuropathy and very easy bruising, etc. this     
just touches the surface of my reactions…oh, forgot when first on Gleevec     
and speaking to Zavie at the beginning, I had a low grade     
temperature…because I have a high tolerance to pain, I did not know I was     
experiencing bone pain…very weird.

AIMS:
      
Imatinib mesylate (Gleevec/Glivec),       which has revolutionized the 
treatment of chronic myeloid leukemias (CML)       and gastrointestinal stromal 
tumours (GIST), has been reported to cause       gastric upset. Consequently, 
proton pump inhibitors (PPI) are frequently       co-administered with 
imatinib. Because PPI can elevate gastric pH and       delay gastric emptying 
or antagonize ATP-binding-cassette transporters,       they could influence 
imatinib absorption and pharmacokinetics. We aimed to       evaluate whether 
use of omeprazole has a significant effect on imatinib       pharmacokinetics.
      
METHODS:
      
Twelve healthy subjects were enrolled       in a two-period, open-label, 
single-institution, randomized cross-over,       fixed-schedule study. In one 
period, each subject received 400 mg imatinib       orally. In the other 
period, 40 mg omeprazole (Prilosec) was administered       orally for 5 days, 
and on day 5 it was administered 15 min before 400 mg       imatinib. Plasma 
concentrations of imatinib and its active N-desmethyl       metabolite CGP74588 
were assayed by LC-MS, and data were analyzed       non-compartmentally.
      
RESULTS:
      
PPI administration did not       significantly affect the imatinib area under 
the plasma concentration vs       time curve (AUC) (34.1 microg ml(-1) h alone 
vs 33.1 microg ml(-1) h with       omeprazole, P= 0.64; 80% power), maximum 
plasma concentration (C(max))       (2.04 microg ml(-1) alone vs 2.02 microg 
ml(-1) with omeprazole, P= 0.97),       or half-life (13.4 h alone vs 14.1 h 
with omeprazole, P= 0.13).
      
CONCLUSIONS:
      
Our results indicate that the use of       omeprazole does not significantly 
affect the pharmacokinetics of imatinib,       as opposed to, for example, 
dasatinib where PPI decreased AUC and C(max)       two-fold.
      
 
      
Don't know when this study was done, but it seems       Gleevec is different 
than Sprycel.  I was on Gleevec for 5 years, and       I do think it affects 
the stomach.
      
I       was on the proton pump inhibitor while in the hospital but when I went 
on       Sprycel, the leaflet said no, so I quit them.  They never seemed to    
   help me anyway.

That is so strange; you aren't supposed to be taking those proton         pump 
inhibitors at all at least that's what is says on my Sprycel         leaflet; 
Check you leaflet for drug not to take with your meds.

Silent reflux just means I have no symptoms...no heartburn BUT           
experience hoarseness and other symptoms. 
          


          
Dr. Druker asked me to try prilosec.  I did and was off it           in two 
days...the muscle spasms were worse than the Gleevec           spasms...never 
imagined anything worse...but it was. ..up all night           except for 3 
hours of sleep.
          


          
I wrote Dr. Druker after reading about side effects on the           internet.  
He then wanted me to try Nexium...told him I'm done           being a guinea 
pig...would try diet eating  alone

Hi Susan and so sorry to hear about your reflux.  Be careful           about 
proton pump inhibitors as you are not supposed to be on those           when on 
your TKIs as least that is what my Sprycel booklet           states.  It says 
you can take antiacids 2 hours before or 2 hours           after your meds.  
This is things like Tums.
          
No, no hoarseness, but my ear fills full and my jaw hurts at the           
point of the hinge.
          
What is silent reflux?
