I think that what Lennart meant was that the CMS is often tightly integrated
with the webserver. Two examples I know where this is the case are Vignette
and Microsoft CMS products, both of which intercept use an ISAPI filter to
intercept the HTTP request from the browser before the webserver gets a
chance to deal with it in the normal way (i.e. when the webserver is serving
static HTML files from its filesystem). In this instance there is not
communication between the webserver and CMS as such.

If the integration between the webserver and the CMS isn't so tight, perhaps
the CMS generates static files, then other methods of outputting pages to
the webserver might be via the filesystem, FTP, SOAP the list goes on. It is
very much determined by the CMS and the implementation.

If the CMS is dealing with remote servers (perhaps partner or customer
servers on another network) then the possibilities are even greater as some
systems support content echange standards, such as ICE, RosettaNet and even
EDI.
