I agree with your assessment of why they recommend against extensions.  I used to use a 4:1 multiplier to torque fuse bolts on bomb bodies (650 foot-lb).  It is a true challenge to support the multiplier without an extension.  With an extension, I strongly agree with using a support.  The other challenge that was that the extension breaker bar was really hard on the hands to hold.  If you find the multiplier hard to resist, putting the multiplier in the bilge might be easier.  You might want to be prepared for either option.
