Yeah Johh, my thinking is that that is the biggest concern anytime an extension or adapter is being used. The further off axis the head of the wrench the more inaccurate the torque being applied. A more purposeful statement to that effect in the multiplier instructions would have been nice. With a multiplier any introduced error is amplified... In this case times 3.
