SubjectsPlus (an open-source subject guide system built by Code4Libber Andrew Darby) generates a nice A-Z list. I don't know if you're looking for a solution for subject guides, too, but if you are I'd definitely give that a look -- http://www.subjectsplus.com. We use it here and like it a lot.
