Review comment:
Yes. The previous primary key was not named task_instance_pkey, it was
named `PK__tas.....` can't remember the full name but what I found out is, the
columns(dag_id, task_id, and execution_date ) were all non-nullable in mssql
before the upgrade that created `task_instance_pkey`
