#### Refactor ControlClientPool to allow client multiplexing
InstructionRequestHandlers (control clients) are now identified by worker id
on both the producer and consumer sides. Note that the sdk harness does not
populate request headers with worker ids, so anywhere the harness is invoked
_outside_ of the container context where the go boot code sets this up does not
have worker ids available and instead identifies all workers by `null`.
#### Basic Docker environment factory
Implements a basic environment factory that shells out to docker and
terminates the docker process when the environment handle is closed.
