Review Comment:
   So if we reach this point and then fail, the on-disk v3 file would have been 
appended with the dictionary (and metadata will reflect that dictionary exists) 
but forward index will not have been rewritten to be dictionary based at this 
point.  
   
   Is this handled as part of the overall failure handling framework during 
reload / load ?
