Uh, one day I discovered that iCab ppc was unable to collect the
"OPT-OUT" cookie available for free from doubleclick. I wrote to iCab.
The final exchanges follow:

Maybe a certain iCab setting will filter out this filter. In case the
cookie is not fully legal or valid, it can be automatically filtered out
with a certain cookie setting of the preferences dialog.

The only problem is that doubleclick doesn't send this cookie to iCab.
So if iCab doesn't receive this cookie it can't store and use it.

I've now checked this so I know why this is happening. The problem is the
default "Referer" setting (Preferences > Network > HTTP) of iCab. iCab
will use a more secure "Referer" setting so that the Referer is only
within the same domain. But because when setting the "opt-out" cookie
the server/domain will change and the "Referer" is not send, so it
doesn't work. If you configure iCab to always send the "Referer" (which
is the default setting of Netscape and MSIE, but which can be also a real
security risk and therefore many firewalls do filter out the "Referer"
information as well) you'll get the opt-out cookie with iCab as well.

================

Maybe not, but perhaps this info can help some iCab fans here achieve
better performance.
