As close as possible to the source of heat (ie analog board and psu), 
in theory. However if you mount the fan assembly inside the case, 
you'll have to do some modification, even just gluing some mounts in 
place or hanging some brackets off the a/b. The alternative is to put 
the fan assembly on the outside, drawing air through the vents on the 
top. Not particularly good looking, I agree, but a much better solution 
than cooking the Mac and you can easily remove it.

For an internal fan, tap 12V from the psu. But the Plus psu may be 
marginal anyway so do you want to add a couple of watts on top? For an 
external fan, you have 220/240V to play with so you can afford to suck 
a lot of air through the Plus to cool it. The mains powered Kensington 
System Saver on my IIgs draws a lot more air than it needs to.

In general, you should look to creating a smooth path to draw air out 
of a computer than blowing it in. It took the designers of PC ATX 
systems a while to work this out; in many designs, early ATX psus were 
designed to blow warm air from the psu onto the processor... For a Mac 
Plus, it should be clear that there is no point blowing air into the 
case from (near) the top because there is no clear exit path.

My own philosophy towards potential overheating of a 128/512/Plus is:

1. Don't worry too much if you have spare boards and a willingness to 
wield a soldering iron.
2. Don't leave them running when not actively in use.
3. Don't leave them running unattended.
