I can answer two of the questions, at least.
The Mac Plus did not originally come with a fan. It was the last one to do so until the iMac came along. One of the many reasons it's popular.
The last question I found in pickle's FAQ. The signatures inside the case are present in all Macs up to the Plus. It's the signatures of the original development team. Some SEs and SE/30s have them as well, but not all. The Classic and Classic II don't have them.
I don't know any details about the power supply, but I am willing to bet on it being a replacement. I do remember reading some time back that the Plus power supply just isn't powerful enough to handle the draw of a hard drive. It was never intended to have one installed(and is generally not recommended). I'm assuming if it isn't there now, it probably had a hard drive at some point if there's an internal fan. Don't quote me on this answer though. ^^

* finally: I noticed that the case cover has some internal "signatures" marked in high in the plastic. Do all Mac Pluses have these signatures? Unfortunately my camera was not able to capture these details.
