It could be a misaligned or bent pin in the print head. It'd explain the
one-off issue.

A quick way to see is to test the pins individually by printing straight
horizontal lines a pixel wide (somehow figuring out how to get each pin
to fire off) or a page of a checkerboard pattern (like the desktop for a
classic mac). A bent or loose pin will be very apparent on a pixel-sized
checkerboard pattern.
