Just for the record, and in case anyone else hits this problem, I
found a fix.  Upgrading the BIOS and graphics driver to the latest
versions had absolutely no effect.  However all I had to do to fix it
was increase the display resolution from 1024x768 to either 1152x864
or 1280x1024.  So it looks like a driver bug as you said, just at the
1024x768 setting.  This is no doubt hardware dependent (it's a Dell
OptiPlex 790 with i5-2400 CPU and Integrated Intel HD 2000 Graphics).
