Use strace.

(1) telnet localhost 143

(2) In another window, locate the pid of the imapd process, then
        strace -fp <pid>

(3) In first window, issue "A STARTTLS"

(4) Back in the strace window, see what happened. e.g. it may have failed
    to execute couriertls, or failed to open the certificate file.

If you need help interpreting the strace output, capture it to a file, stick
it up on a webserver, then post the URL here.
